New Patient Onboarding Checklist (Required before your first visit)

Step 1 — Complete Your Profile

On the FollowMyHealth portal, Go to:

My Account → My Info

Complete:

☐ General Information (Required) 
Please complete the following fields: Full Name, Date of Birth, Phone Number, Email, Address, Birth Sex, Preferred Pharmacy. 

☐ Emergency Contact (Recommended)

☐ Responsible Party (if applicable)

☐ Insurance Information 
Right Place Health is currently an out-of-network practice. Adding your insurance information helps if you later request a superbill or your provider orders referrals, medications, laboratory tests, or imaging.

Step 2 — Review Communication Preferences

Go to:

My Account → Preferences →  Communication Preferences

We recommend enabling both Email and Text Messages so you receive appointment reminders and important updates.

Step 3 — Complete Required Forms

Complete every Required form listed on your FollowMyHealth Home page. 

Step 4 — You're Ready

You're Ready for Your Appointment! Once Steps 1–3 are complete, no further paperwork is needed before your appointment.
