Our Services

Our services include:

  • Preventive services may include:

    • Annual physical exams

    • Blood pressure screening

    • Cholesterol screening

    • Diabetes screening

    • Thyroid screening when indicated

    • Cancer screening guidance (breast, cervical, colon, prostate)

    • Immunization review and recommendations

    • Cardiovascular risk assessment

    • Lifestyle and nutrition counseling

  • We provide several women’s health services including:

    • Pap smears

    • Evaluation of menstrual concerns

    • Menopause symptom management

    • Breast health concerns

    • Contraception counseling

  • Men’s health services may include:

    • Prostate health evaluation

    • Erectile dysfunction

    • Testosterone evaluation when clinically indicated

    • Cardiovascular risk assessment

    • General health screening

  • Weight management is individualized and may include:

    • Assessment of medical history and contributing factors

    • Evaluation for conditions such as diabetes, thyroid disorders, and metabolic syndrome

    • Lifestyle counseling, including nutrition and physical activity

    • Behavioral strategies for sustainable weight management

    • Medication options when clinically appropriate

  • We provide evaluation and initial management for common mental health concerns, including:

    • Depression

    • Anxiety

    • Stress-related concerns

    • Sleep-related issues


  • Depending on the clinical situation, we may order:

    • Laboratory testing

    • Imaging studies (X-ray, ultrasound, CT, MRI)

    • Cardiac monitoring (heart rhythm monitoring when needed)

    • Home sleep studies for evaluation of sleep apnea

    These tests are performed through independent laboratories and diagnostic facilities.

  • Evaluation and treatment for new symptoms such as:

    • Fever, cough, or respiratory infections

    • Sore throat or sinus symptoms

    • Urinary tract infections

    • Rashes or skin concerns

    • Headaches or migraines

    • Stomach pain or digestive symptoms

    • Minor injuries or musculoskeletal pain


  • Follow-up and management for conditions such as:

    • Hypertension

    • Diabetes

    • High cholesterol

    • Thyroid disorders

    • Asthma

    • Migraines

    • GERD

    • Chronic pain conditions

  • When additional evaluation or specialized care is needed, we may refer patients to appropriate specialists or services, including:

    • Cardiology

    • Endocrinology

    • Gastroenterology

    • Gynecology

    • Dermatology

    • Mental health providers (therapy and psychiatry)

    • Physical therapy

    • Nutrition services

    • Massage therapy and supportive care services

How We Work

To be seen by our provider please follow this process:

    1. Click Book Appointment button above to request an appointment.

    2. Provide Name, DOB, Phone Number.

    3. We will get back to you to schedule your visit.

    1. Create FollowMyHealth portal account (you will receive a link via email after your appointment is confirmed)

    2. Complete required forms in the FollowMyHealth portal before your appointment.

    Completing pre-visit forms is required and helps the provider provide you the best possible care.

  • Discuss your health concerns with your provider. We provide both in-person appointments and telehealth appointments for your convenience.

  • Our provider will guide you to low-cost options for lab work or imaging services (x-ray, MRI, etc.)

    If you have insurance, you may use it for labs and imaging.

  • Pay for the provider’s consultation at the time of your appointment. Insurance is not accepted for consultations at this time.

    • Consultations fees: $120.

    • Annual Physical appointment: $150.

    You may request a superbill if your insurance covers out-of-network service.
